1 Effect of a theory-driven educational intervention on the level of knowledge, attitudes, and assessment practices regarding breakthrough cancer pain (BTCP) management among medical nurses in Hong Kong

Purpose: To examine the effect of an educational intervention intended to improve medical nurses’ adherence to breakthrough cancer pain (BTCP) assessment practices and their level of knowledge, attitudes and perceived assessment practices regarding BTCP management. Methods: Nurses working in a regional hospital were recruited to this quasi-experimental study. The intervention group received a 3-h educational workshop and one session of individual clinical instruction, whilst the control group did not receive any intervention. Chart audits were performed to examine adherence to BTCP assessment practices as the primary outcome. A self-developed questionnaire was used to measure nurses’ knowledge, at- titudes and perceived assessment practices regarding BTCP management as the secondary outcomes. The chi- square or Fisher’s exact test was used to compare the rate of adherence to BTCP assessment practices between groups. A generalised estimating equation was used to compare changes in knowledge, attitudes, and perceived assessment practices between groups over time. Results: One hundred and five nurses completed the study. The chart audits revealed a significantly higher rate of adherence to BTCP assessment practices in the intervention group after the intervention (p < .05). The inter- vention group exhibited significant positive changes in scores for knowledge (β = 25.49, p < .001), attitude (β = 0.98 to 2.81, p < .01), and their perceived assessment practices (β = 1.33 to 3.14, p < .002) when compared with the control group. Conclusions: This theory-driven educational intervention significantly improved the medical nurses’ adherence to BTCP assessment practices and their level of knowledge attitudes and perceived assessment practices regarding BTCP management.

2 Factors Relating to Nurses’ Knowledge and Attitudes Regarding Pain Management in Inpatients

Purpose: To describe factors associated with nurses’ attitudes or lack of knowledge regarding pain management in adult inpatients.
Design: Transverse descriptive survey-based study. Methods: This was a transverse descriptive survey-based study. The population was obtained through nonprobability convenience sampling. The Knowledge and Attitudes Survey Regarding Pain was made available to 470 nurses at a tertiary level hospital. Associations were sought with the unit where assigned, years of experience, specific training on pain, and postgraduate education.
Results: The sample included 134 nurses with a mean age of 41.6 ± 10.8 years; 87% were women, 64% worked rotating shifts, 64% had more than 10 years of experience, and 31% had specific training in pain management. The greatest number of correct responses was obtained from nurses with specific training in pain management (p ¼ .001) and nurses who worked in units of surgical hospitalization (p ¼ .004). The lack of training was associated with a deficit in knowledge and inadequate attitudes about pain management. In nurses with less than 10 years of experience, worse results were observed in knowledge, whereas the unit of work was decisive in the results about attitude (p < .05).
Conclusions: Among the nurses surveyed, some knowledge gaps were detected, as were certain inappropriate attitudes, associated with lack of training, lack of experience, and being assigned to specific hospitalization units.

3 A Nonrandomized Pretest Posttest Study on the Impact of an Educational Pain Management Program on Nurses’ Knowledge and Attitudes Regarding Pain in a Middle Eastern Country

Background: Although nurse’s knowledge and attitudes regarding pain management has been sufficiently studied worldwide, the impact of an educational intervention program in improving such attitudes and knowledge has not been likewise researched, especially in Middle Eastern countries.
Aims: To examine nurses’ knowledge and attitudes regarding pain at a university hospital in Lebanon before and after the introduction of a pain management educational program. And to assess the relationship between the characteristic of nurses and their pain knowledge.
Methods: Design; A nonrandomized pretest posttest study design was used. Setting; A university hospital in Lebanon. Participants; Included 183 nurses using the Nurses’ Knowledge and Attitudes Survey Regarding Pain questionnaire. The pain educational intervention was based on the principles of Ajzen’s theory of planned behavior.
Results: A significant difference between the pre and post test scores was noted (p = .016). Questions answered correctly by 80% of participants were related to questions about pain, pain assessment and management, and questions related to medications, such as correct dosages and opioid side effects were not answered correctly by the majority of nurses. There were significant associations between test scores and the nurses’ educational level, their age, and their years of experience. Nurses who worked in critical care units, the emergency department and oncology had higher scores than nurses who worked on general units.
Conclusions: Despite the intensive pain education provided at our institution, the pain knowledge of nurses remains below that recommended level which indicates a dire need for more intensive and continuous education in order to provide a pain free environment.

4 Educational Intervention to Strengthen Pediatric Postoperative Pain Management: A Cluster Randomized Trial

Background: Pediatric postoperative pain is still undertreated. Aims: To assess whether educational intervention increases nurses’ knowledge and improves pediatric postoperative pain management.
Design: Cluster randomized controlled trial with three measurement points (baseline T1, 1 month after intervention T2, and 6 months after intervention T3). Participants/Subjects: The study was conducted in postanesthesia care units at six hospitals in Norway. Nurses working with children in the included units and children who were undergoing surgery were invited to participate in this study.
Methods: Nurses were cluster randomized by units to an intervention (n = 129) or a control group (n = 129). This allocation was blinded for participants at baseline. Data were collected using “The Pediatric Nurses’ Knowledge and Attitudes Survey Regarding Pain: Norwegian Version” (primary outcome), observations of nurses’ clinical practice, and interviews with children. The intervention included an educational day, clinical supervision, and reminders.
Results: At baseline 193 nurses completed the survey (75% response rate), 143 responded at T2, and 107 at T3. Observations of nurses’ (n = 138) clinical practice included 588 children, and 38 children were interviewed. The knowledge level increased from T1 to T3 in both groups, but there was no statistically significant difference between the groups. In the intervention group, there was an improvement between T1 and T2 in the total PNKAS-N score (70% vs. 83%), observed increase use of pain assessment tools (17% vs. 39%), and children experienced less moderate-to-severe pain.
Conclusions: No significant difference was observed between the groups after intervention, but a positive change in knowledge and practice was revealed in both groups. Additional studies are needed to explore the most potent variables to strengthen pediatric postoperative pain management.

5 Empowering seizure management skills: Knowledge, attitudes, and experiences of school staff trained in administering rescue drugs in Northern Italy

Purpose: The administration of rescue medication at school concerns students for which it may be essential, on doctor’s prescription, to take therapy during school hours. In this case, since the parents are absent, the first rescuer is necessarily the school staff, who should be properly trained because prolonged seizures can cause severe harm and even death.
Methods: Every year, the Local Health Unit ‘‘TO3” in Northern Italy, provides training for school staff to administrate rescue medication at school. From December 2019 to February 2020, the same questionnaire was administered to school staff trained for seizures at the end of the course, while the staff trained for other diseases completed it before the course.
Results: About 60% of the sample (N = 123) had been trained in seizure management at least once in their lifetime. Median knowledge score in subjects with no seizures training was 7 (Q25/Q75: 5/8), while it was 9 (Q25/Q75: 6/10) in subjects with seizures training (p < 0.001). The self-reported level of confidence in their skills to administer rescue medication was high in 10.2% of subjects not trained for seizures and in 62.9% of those trained (p < 0.001).
Conclusion: Results suggest that medical training for school staff increases knowledge scores and levels of self-confidence relating to the administration of rescue medication. Moreover, after the training, the school staff changed attitude toward seizures, no longer considering them a problem, and became more aware, less fearful, and more inclined to act in case of need, making school a better place for all students.

6 Self-management on heart failure: A meta-analysis

Background and aims: Heart failure (HF) is a severe public health problem all over the World. Selfmanagement is an effective method to progress self-care ability. However, the role of selfmanagement in heart failure has not been thoroughly elucidated.
Methods: The research articles related to heart failure were searched by the PubMed, Embase, Cochrane databases, and China National Knowledge Database on articles published through March 2020. The average 95% of confidence intervals (CIs) were used to calculate using random-effects or fixed-effects. Review Manager (version 5.2) was adopted for meta-analysis, sensitivity analysis, and bias analysis.
Results: Eight (8) eligible studies with 1707 patients with HF were included in this analysis. In the Metaanalysis showed significant differences for Self-management (SM) groups in Dutch Heart Failure Knowledge Scale (DHFK) (MD ¼ 1.36, 95%CI [-0.03, 2.75], P ¼ 0.04; I2 ¼ 83%), in Self-Care of Heart Failure Index (SCHFI) (MD ¼ 5.51, 95%CI [0.62, 10.40], P ¼ 0.03; I2 ¼ 70%), and in Self-Efficacy for Managing Chronic Disease Scale (SEMCDI) (I2 ¼ 47%, Z ¼ 5.43, P of over effect < 0.0001) than control groups. One bias is detected as attrition bias, and another one is reporting bias. Sensitivity analysis satisfied the stability of the results.
Conclusion: Self-management was associated with significant outcomes in patients with HF through knowledge, attitude, and practice (KAP).

7 The role and meaning of lessons learned in project knowledge management in organizations in Poland

The aim of this article is to establish the role and meaning of lessons learned collection and application practices in project knowledge management based on an empirical research in organizations running operations in Poland. The analysis conducted for the purpose of this article included hypothesis verification by means of statistical testing performed on data gathered from an online survey distributed among project management professionals in Poland. The testing results revealed that Polish project managers and project team members present positive attitude towards lessons learned practices and that there is a statistically significant correlation between lessons learned collection and application practices and benefits realization in terms of scope, time and budget in projects in the surveyed organizations differentiated by means of sector of operations, project maturity level and organizational culture. Also, specific barriers and obstacles in learning lessons from past experience in project management have been found and correlated with lessons learned collection and application practice.

8 Effects of demand uncertainty reduction on the selection of financing approach in a capital-constrained supply chain

This study investigates how demand uncertainty reduction (DUR) affects the decisional dynamics within a supply chain, which comprises a supplier and a capital-constrained retailer, who chooses between bank credit and trade credit financing. A comprehensive scenario analysis suggests the retailer should accept trade credit when DUR is high, trade credit risk premium is moderate, and wholesale price is exogenous and low. However, the retailer should adopt trade credit only when both DUR and production cost are not high, and wholesale price is set endogenously. We further relax the assumption on the bank’s risk attitude and find most results still hold.

9 Disaster management training in the euregio-meuse-rhine: What can we learn from each other to improve cross-border practices?

Increasing numbers of disasters require comprehensive preparedness. Border regions are vulnerable as disasters might not halt at administrative borders. Cross-border coordination is therefore required. As integral part of cross-border collaborations initiative in the Meuse–Rhine Euregio (EMR), we reviewed published evidence informing on existing initiatives dedicated to disaster education in the EMR. A search based on the PRISMA guidelines for scoping reviews was conducted to retrieve articles in the following databases: Medline, PsychInfo and Scopus. The searches were limited to English, French, Dutch and German language articles and the period between January 2010 and June 2019. No restrictions were set for the study design or the type of methodology used. A total of 18 articles met the inclusion criteria out of a total of 1771 publications. Training development was found in two studies while nine studies focused on the state of knowledge in disaster management. Seven articles referred only to technical skills, three only to non-technical skills and eight combined both types of skills. For the technical nature, Knowledge was found seven times, Skills five times and Attitudes twice. On the non- technical side, Knowledge was found three and both Skills twice and Attitudes three times. Five studies trained and assessed all the Knowledge, skills and attitudes. Most of the studies constitute inventories with descriptive reporting and very few experimental studies of quality have been carried. Non-technical skills for disaster preparedness have been well considered among the articles. Cross-border collaboration needs to be further investigated.

10 Knowledge, attitudes and practices regarding prediabetes among general practitioners in Reunion Island

Background: General practitioners (GPs) are among the first health care providers to be involved in prediabetes care, and their actions can prevent patients’ transition from prediabetes to diabetes. However, data on knowledge and management of prediabetes in primary care remain scarce. Aim: To describe the knowledge, attitudes and practices of GPs towards prediabetes. Design and setting: This descriptive cross-sectional study included all private GPs working in Reunion Island.
Method: GPs were sent an electronic questionnaire that specifically sought to establish whether GPs knew the definition of prediabetes and whether their practices were in line with French guidelines for prediabetes screening and management.
Results: A total of 121 GPs completed the questionnaire. Of these, 95.0% knew the definition of impaired fasting glucose, but only 66.1% knew that of impaired glucose tolerance. Many GPs reported practices that are not recommended in guidelines. Thus, 59% of respondents declared that they monitored prediabetes with HbA1c and 10% said that they prescribed oral anti-diabetic drugs as prediabetes treatment. The risks factors associated with prediabetes were generally well known. Although 59.5% of respondents reported not having enough time to provide therapeutic education and medical follow up, only 19% referred their patients to a nutritionist or dietician. Lastly, 93.4% of respondents favoured the implementation of a local therapeutic education program targeting prediabetic individuals.
Conclusion: Prediabetes management in primary care can be made more effective by improving GPs’ knowledge of prediabetes and official guidelines, facilitating referral of prediabetic patients to lifestyle specialists and implementing local therapeutic education programs.
