1 LAMD: Location-based Alert Message Dissemination scheme for emerging infrastructure-based vehicular networks

Requiring low dissemination delays and thorough vehicles coverage in the vicinity of an emergency event, Vehicular Ad-hoc NETworks (VANETs) were considered as the most adapted communication network to support alert messages dissemination. With the advent of Cooperative ITS services, emerging vehicular networks are expected to increasingly rely on Vehicle to Infrastructure (V2I) communication links, which are expected to be nominally available, with some transient and time-limited connectivity losses. The presence of V2I links paves the way to centralized network control, which can leverage vehicle-related and contextual information provided by the cloud to make more informed decisions. This paper proposes an effective alert message dissemination procedure called LAMD (Location-based Alert Messages Dissemination) for emerging vehicular networks that combines V2I broadcasts with selected V2V (Vehicle to Vehicle) rebroadcasts. The originality of our scheme lies in the selection process of V2V rebroadcasts, which is based on vehicles’ location regarding predefined rebroadcast points selected by a centralized controller. This leads to very limited collisions, low delivery delays, and high information coverage with insignificant signaling and network overhead compared to legacy VANET based dissemination techniques.

2 A Nonrandomized Pretest Posttest Study on the Impact of an Educational Pain Management Program on Nurses’ Knowledge and Attitudes Regarding Pain in a Middle Eastern Country

Background: Although nurse’s knowledge and attitudes regarding pain management has been sufficiently studied worldwide, the impact of an educational intervention program in improving such attitudes and knowledge has not been likewise researched, especially in Middle Eastern countries.
Aims: To examine nurses’ knowledge and attitudes regarding pain at a university hospital in Lebanon before and after the introduction of a pain management educational program. And to assess the relationship between the characteristic of nurses and their pain knowledge.
Methods: Design; A nonrandomized pretest posttest study design was used. Setting; A university hospital in Lebanon. Participants; Included 183 nurses using the Nurses’ Knowledge and Attitudes Survey Regarding Pain questionnaire. The pain educational intervention was based on the principles of Ajzen’s theory of planned behavior.
Results: A significant difference between the pre and post test scores was noted (p = .016). Questions answered correctly by 80% of participants were related to questions about pain, pain assessment and management, and questions related to medications, such as correct dosages and opioid side effects were not answered correctly by the majority of nurses. There were significant associations between test scores and the nurses’ educational level, their age, and their years of experience. Nurses who worked in critical care units, the emergency department and oncology had higher scores than nurses who worked on general units.
Conclusions: Despite the intensive pain education provided at our institution, the pain knowledge of nurses remains below that recommended level which indicates a dire need for more intensive and continuous education in order to provide a pain free environment.

3 Empowering seizure management skills: Knowledge, attitudes, and experiences of school staff trained in administering rescue drugs in Northern Italy

Purpose: The administration of rescue medication at school concerns students for which it may be essential, on doctor’s prescription, to take therapy during school hours. In this case, since the parents are absent, the first rescuer is necessarily the school staff, who should be properly trained because prolonged seizures can cause severe harm and even death.
Methods: Every year, the Local Health Unit ‘‘TO3” in Northern Italy, provides training for school staff to administrate rescue medication at school. From December 2019 to February 2020, the same questionnaire was administered to school staff trained for seizures at the end of the course, while the staff trained for other diseases completed it before the course.
Results: About 60% of the sample (N = 123) had been trained in seizure management at least once in their lifetime. Median knowledge score in subjects with no seizures training was 7 (Q25/Q75: 5/8), while it was 9 (Q25/Q75: 6/10) in subjects with seizures training (p < 0.001). The self-reported level of confidence in their skills to administer rescue medication was high in 10.2% of subjects not trained for seizures and in 62.9% of those trained (p < 0.001).
Conclusion: Results suggest that medical training for school staff increases knowledge scores and levels of self-confidence relating to the administration of rescue medication. Moreover, after the training, the school staff changed attitude toward seizures, no longer considering them a problem, and became more aware, less fearful, and more inclined to act in case of need, making school a better place for all students.

4 Emergency care providers perspectives of acute pain assessment and management in the prehospital setting, in the Western Cape, South Africa: A qualitative study

Introduction: A growing body of evidence suggests that pain knowledge and management are poor, perhaps more so in the prehospital setting. The daily challenges that emergency care providers face in dealing with prehospital pain remain unclear. This study aimed to gain a deeper understanding of acute prehospital pain assessment and management in the Western Cape, South Africa. Methods: A series of focus group discussions, using a constructivist paradigm and qualitative content analysis were conducted. Results: The key themes emerging from six focus groups (total 25 emergency care providers) related to the difficulties of assessing pain in this setting, factors affecting clinical reasoning in this (hostile) setting, the re- alities of prehospital pain care for non-advanced life support practitioners, along with emergency departments’ lack of understanding and appreciation of the prehospital environment, and participants’ suggestions to improve pain practice. Conclusion: Several barriers and enablers, some novel, to pain assessment and management in the South African prehospital setting were identified. Our findings provide valuable insight and understanding of the challenges related to pain care prehospital providers face, in other similar prehospital settings, but also to the global body of knowledge on prehospital barriers and enablers of pain assessment and management.

5 Midwives knowledge of pre-eclampsia management: A scoping review

Background: Pre-eclampsia is a multi-organ disease affecting pregnant women from the second trimester onwards resulting in multiple adverse outcomes. Sub-optimal treatment of pre-eclampsia is linked with unfavorable outcomes. It is critical for midwives as primary providers to be competent in the diagnosis and management of pre-eclampsia especially in low-and middle-income countries.
Aim: To identify what midwives’ around the world know about pre-eclampsia management. Methods: A scoping review using the JBI three-step search strategy was used to identify relevant research articles and grey literature on the subject. Database searches in PubMed, CINAHL, Cochrane Databases, Web of Science, and Scopus yielded twenty papers in addition to nine guidelines from Google Scholar. The findings were synthesised using a metasynthesis approach and presented as themes.
Findings: Four themes were identified from the extracted data: Foundational knowledge of preeclampsia; Knowledge and management of a woman with pre-eclampsia according to guidelines; Knowledge of being prepared for emergency procedures and management of emergencies; Factors influencing knowledge. The first three themes addressed diagnosis and management whilst the last theme described how contextual factors led to either increased or decreased knowledge of preeclampsia.
Conclusion: Worldwide, practicing midwives lack knowledge on several aspects of pre-eclampsia diagnosis and care. Policies on in-service training should be oriented to include innovative nontraditional methods that have the potential to increase midwives’ knowledge.

6 Knowledge of healthcare providers in the management of anaphylaxis

Introduction: Anaphylaxis is defined as a severe, life-threatening systemic hypersensitivity reaction. Early diagnosis and treatment of a severe allergic reaction requires recognition of the signs and symptoms, as well as classification of severity. It is a clinical emergency, and healthcare providers should have the knowledge for recognition and management. The aim of the study is to evaluate the level of knowledge in the management of anaphylaxis in healthcare providers.
Methods: It is an observational, descriptive, cross-sectional study conducted among healthcare providers over 18 years old via a Google Forms link and shared through different social media platforms. A 12-item questionnaire was applied which included the evaluation of the management of anaphylaxis, from June 2020 to May 2021.
Results: A total of 1023 surveys were evaluated; 1013 met inclusion criteria and were included in the statistical analysis. A passing grade was considered with 8 or more correct answers out of 12; the overall approval percentage was 28.7%. The group with the highest percentage of approval in the questionnaire was health-care providers with more than 30 years of work experience. There was a significant difference between the proportions of approval between all specialty groups, and in a post-hoc analysis, allergy and immunology specialists showed greater proportions of approval compared to general medicine practitioners (62.9% vs 25%; p¼<0.001).
Conclusions: It is important that healthcare providers know how to recognize, diagnose, and treat anaphylaxis, and later refer them to specialists in Allergy and Clinical Immunology in order to make a personalized diagnosis and treatment.

7 Knowledge in pre-hospital emergency and risk management among outdoor adventure practitioners in East Africa afro-alpine mountains

Introduction: The enjoyment of nature and other benefits of outdoor activities happen amid inherent hazards. This calls for knowledge and competency in emergency and risk management. Practitioners in outdoor activities, such as mountaineering, thus need to be knowledgeable on how to manage risks and attend to emergencies in their practice. The study sought to establish the preparedness of East African mountaineering practitioners in pre- hospital emergency and risk management. It sought to establish their knowledge on prehospital emergency and risk management, based on their age, gender, level of education and refresher training. Methods: The study purposively sampled one hundred and thirty six (N = 136) outdoor adventure practitioners from the Afro-alpine mountain areas in East Africa. It was hypothesized that there would be no significant relationship between the outdoor practitioners’ knowledge in prehospital emergency risk management and their age, gender, level of education, refresher training. Somers’ d was used to test the hypotheses. Results: It was established that the knowledge scores of prehospital emergency and risk management for the mountaineering practitioners was low. It was also established that the knowledge scores of outdoor practitioners were not dependent on their age, gender, and work experience. However, there was a significant relationship between the outdoor adventure practitioners’ knowledge scores and their highest level of education as well as refresher training. Conclusions: The study concluded that there were gaps in the knowledge of prehospital risk management of the East African Afro-alpine mountaineering practitioners. It recommends frequent and regular training and re- certification among outdoor adventure practitioners in order to raise the knowledge in prehospital emergency risk management. 8 The optimal recovery-fund based strategy for uncertain supply chain disruptions: A risk-averse two-stage stochastic programming approach

For a supply chain subject to uncertain production disruptions, the joint optimization of invest- ment intervention on recovery speed and duration of disrupted production capacity and location and inventory management has not been well studied. In this paper, a novel recovery strategy is introduced and studied, which uses investment to adjust the recovery speed and duration of production capacity, and two recovery behaviors responding to different types of disruptions are modeled. Considering uncertain disruption scenarios and their ripple effects over the supply chain, a risk-averse two-stage stochastic programming model (RTSPM) is established to study the integrated supply chain management of selection of distribution centers, multi-period inventory, transportation flows, and recovery-fund based mitigation policy. The RTSPM incorporates the risk preference of managers in decision making. We propose a trust-region-based decomposition method to solve the RTSPM and demonstrate its efficiency by benchmarking on state-of-the-art commercial solvers. Through numerical examples, we deeply analyze the effectiveness of RTSPM and the relations of optimal recovery investment decisions with the uncertain disruption factors. Finally, we provide implications and suggestions induced from the models and findings to aid the decisions on renting of distribution centers and the emergency investment and operational decisions when suffering the disruptions.

9 Inventory management in blood supply chain considering fuzzy supply/demand uncertainties and lateral transshipment

Supply and demand uncertainties combined with very short lifetime of blood platelets has led to significant wastage of the total blood collected from the donors. Conversely, great shortage of platelets may be obtained due to the limited number of donors and emergency demands. Therefore, it is of utmost importance to develop appropriate inventory management model to simultaneously minimize both shortage and wastage along the blood supply chain. To achieve this purpose, this paper presents an Inventory Management model for Age- differentiated platelets under supply/demand Uncertainties (IMAU) for Blood Supply Chains with Lateral Transshipment (BSCLT), resulting a new model named IMAU-BSCLT. The proposed model is solved using whale optimization algorithm considering the costs of ordering from blood centers and lateral transshipment, transportation, inventory holding, shortage, and wastage. In order to validate the proposed methodology, a case study of blood supply chain is used to show the usability of the proposed model and claim its benefits over existing models. Simulation results demonstrate that lateral transshipment between different demand nodes has a major impact on load balancing leads to simultaneously reduce both shortage and wastage costs. According to the obtained results, shortage rate (total shortage per total demands) and wastage rate (total wastage per total supply) of the proposed method are 3.4 % and 4.8 %, respectively.

10 A supply chain disruption recovery strategy considering product change under COVID-19

A recent global outbreak of Corona Virus Disease 2019 (COVID-19) has led to massive supply chain disruption, resulting in difficulties for manufacturers on recovering their supply chains in a short term. This paper presents a supply chain disruption recovery strategy with the motivation of changing the original product type to cope with that. In order to maximize the total profit from product changes, a mixed integer linear programming (MILP) model is developed with combining emergency procurement on the supply side and product changes by the manufacturer as well as backorder price compensation on the demand side. The model uses a heuristic algorithm based on ILOG CPLEX toolbox. Experimental results show that the proposed disruption recovery strategy can effectively reduce the profit loss of manufacturer due to late delivery and order cancellation. It is observed that the impact of supply chain disruptions is reduced. The proposed model can offer a potentially useful tool to help the manufacturers decide on the optimal recovery strategy whenever the supply chain system experiences a sudden massive disruption.
