1 Effects of a symptom management intervention based on group sessions combined with a mobile health application for persons living with HIV in China: A randomized controlled trial

Objective: This study aims to evaluate the effects of a symptom management intervention (SMI) based on symptom management group sessions combined with a mobile health (mHealth) application (app) on the knowledge of symptom management, the certainty of symptom self-management, symptom severity, symptom distress, medication adherence, social support, and quality of life among persons living with HIV (PLWH) in China.
Methods: A parallel randomized controlled trial with 61 PLWH was conducted in Shanghai, China. The participants in the control group (n ¼ 30) downloaded the Symptom Management (SM) app according to their needs and preferences, and received routine follow-ups. The participants in the intervention group (n ¼ 31) were guided to download and use the SM app, and received four tailored weekly group sessions at routine follow-ups. Each group session lasted for approximately 2 h and targeted one of the major modules of the SM app. All the outcomes were assessed at baseline and post-intervention. The study was registered with the Chinese Clinical Trial Registry (ChiCTR1900024821).
Results: The symptom management knowledge and certainty of symptom self-management were significantly improved after the intervention (all P < 0.01). Compared with the control group, the scores of symptoms reasons knowledge score improved 11.47 points (95% CI: 3.41, 19.53) and scores of symptoms self-management knowledge score improved 12.80 points (95% CI: 4.55, 21.05) in the intervention group after controlling for covariates. However, other outcomes did not show statistically significant differences between the intervention group and the control group (P > 0.05).
Conclusion: The SMI could improve PLWH’s symptom management knowledge and certainty of symptom self-management. Multi-center studies with larger sample sizes and longer follow-ups are needed to further understand the effects of SM app on ameliorating symptom severity and symptom distress. More innovative strategies are also needed to promote and maintain the sustainability of the SM app.

2 A literature review and meta-analysis on the effects of ADHD medications on functional outcomes

Objective: To conduct a systematic review and meta-analysis of literature from large databases and registries to assess the effects of ADHD medication on associated functional outcomes. Study design: A literature search was performed in PubMed, PsycINFO, MEDLINE, and Web of Science for articles published prior to January 2019. Sample size, age range, country of origin, medication type, number of functional events and non-events, odds ratios and hazard ratios, and means and standard deviations were extracted. Random-effects meta-analyses were conducted for 21 studies examining functional outcomes. Results: 40 articles were included. The majority suggest a robust protective effect of ADHD medication treatment on mood disorders, suicidality, criminality, substance use disorders, accidents and injuries, traumatic brain injuries, motor vehicle crashes, and educational outcomes. Similarly, the meta-analyses demonstrated a protective effect of medication treatment on academic outcomes, accidents and injuries, and mood disorders. Conclusions: These findings suggest that ADHD medication treatments are associated with decreases in the risks for a wide range of ADHD-associated functional outcomes supporting efforts aimed at early diagnosis and treatment of individuals with ADHD.

3 CONSIGNELA: A multidisciplinary patient-centered project to improve drug prescription comprehension and execution in elderly people and parkinsonian patients

Many older patients and patients with Parkinson’s disease (PD) do not respect medication pre scriptions. Non-adherence is caused by several factors among which three play an important role: Treatment complexity, cognitive decline, and patient-provider communication. This article presents an ongoing multidisciplinary project, the CONSIGNELA project, which adopts a patient centered approach for improving medication adherence in these patients. The first objective of the project is to identify the best way to communicate medication prescriptions with tablets and touch-screen devices. The second objective is to improve medication adherence by creating a collaborative application (app) designed both for older and PD patients and for healthcare professionals in charge of them. Two solutions are combined: A cognitive and a technological solutions. The cognitive solution consists in studying in real time older and PD patients while consulting and executing prescriptions presented in different formats to select the most efficient one. The technological solution aims to improve collaboration between health professionals and patients by developing System of Systems (SoS). The research model used in the project includes three phases. Only the results of phase 1 are presented in the article. A research app, CONSIGNELA-Appli-R was programmed to analyze patient cognitive processes while consulting a prescription on a tablet. A pilot study was carried out with young adults to test it. It confirmed a facilitating effect of table format. In parallel, the prototype of a second app, CONSIGNELA-Appli P, for patients and healthcare providers was developed. It is based on an SoS architecture con necting a virtual pillbox, a Multi-Agent System (MAS) and a knowledge platform. The virtual pillbox is used by patients and providers to improve mutual understanding of medication pre scriptions. The MAS provides agents that extract and analyze indicators, actions and information resulting from the interaction between patients and virtual pillboxes. All the information is ca pitalized and stored in the knowledge platform. Providers can receive charts and dashboards showing information about the following of the medication regimen, for instance, if patients are following it correctly or not.
