1 Pain management for infants – Myths, misconceptions, barriers; knowledge and knowledge gaps
مدیریت درد برای نوزادان - افسانه ها، باورهای غلط، موانع؛ دانش و شکاف دانش-2021
Twelve years ago, the paper ‘Oral sucrose for pain management in infants: Myths and misconceptions’ was published in the Journal of Neonatal Nursing. At this time, eight myths or misconceptions were addressed. Since this time there has been more than 100 studies published reporting on analgesic effects of sweet solutions in newborns, which have been synthesised and included in systematic reviews. There has also been a growth of literature to support analgesic effects of breastfeeding and skin-to-skin care as well as concerning evidence of adverse long-term neurobehavioural outcomes associated with painful procedures. Yet, ongoing studies of pain management practices continue to report inconsistent use of these strategies during painful procedures. We are therefore at a cross-roads of evidence – there is knowledge of effective treatments, knowledge of harm of untreated pain, yet sick, premature as well as healthy infants are continuing to be exposed to painful procedures with no effective treatment. There are however ongoing myths, misconceptions, and practical barriers to using the evidence as well as ongoing knowledge gaps. This paper will therefore highlight existing myths, misconceptions, barriers and current knowledge gaps to using the three key evidence-based pain management strategies of breastfeeding, skin-to-skin care and sweet solutions, in the hope that this will bring to light newborn infant pain treatment practices that can be improved.

2 Nurses perspectives on pain management practices during newborn blood sampling in China
دیدگاه های پرستاران در مورد شیوه های مدیریت درد در طی نمونه گیری خون نوزادان در چین-2021
Introduction: Nurses’ use of evidence-based pain treatments for newborns during needle-related procedures in China was unknown. This study aimed to ascertain knowledge and use of pain management strategies and usefulness of a publicly accessible ’BSweet2Babies’ video, produced in Mandarin, demonstrating the use of breastfeeding, skin-to-skin care (SSC), and sweet solutions during painful procedures. Methods: An online survey was conducted during six nursing conferences in China ascertaining nurses’ previous viewing of the video and knowledge and use of the demonstrated strategies. Results: 221 nurses participated. Only 25 (11.3%) had previously seen the video. Over half knew that breast- feeding (n = 138, 62.4%) and SSC (n = 173, 78.3%) reduced pain, and 89 (40.3%) knew that sucrose reduced pain, but these strategies were infrequently used. Most intended to use the strategies in the future. Discussion: A knowledge-to-action gap for newborn pain management was identified. Future research is needed to improve the implementation of effective pain treatment for newborns.

3 Management of blood products: Nursing knowledge and practices at an academic hospital
مدیریت محصولات خون: دانش و شیوه های پرستاری در یک بیمارستان دانشگاهی-2021
Objective: The purpose of this study was to determine the knowledge and practice of nurses at an academic hospital regarding the handling of red blood cells (RBCs) and platelets. Methods: A descriptive study was performed using a paper-based questionnaire. Nurses from 12 wards where these products are frequently transfused were purposefully selected to participate. Results: Ninety-nine questionnaires were analysed. While 78.9 % respondents reported that their knowledge regarding the handling of RBCs and platelets was adequate, 81.7 % still stated that they would like more training on this subject. Almost half (45.2 %) reported that the time of arrival of RBCs and platelets on the ward was not charted, while 25.5 % indicated that they warm all these blood products before transfusion. About eighty percent (80.8 %) did not know where to store platelets in the ward and 37.4 % knew the average duration of a platelet transfusion. The term, blood on returnable basis was correctly interpreted by 6.1 % of respondents. Conclusion: Gaps exist in the knowledge and practice of nursing staff regarding the correct handling of RBCs and platelets, specifically discarding, storage and warming of these products. Addressing such gaps would improve patient care and contribute towards patient blood management.

4 Pain Prevalence, Pain Management, and the Need for Pain Education in Healthcare Undergraduates
شیوع درد، مدیریت درد و نیاز به آموزش درد در دانشجویان مراقبت های بهداشتی-2021
Background: Pain is a common health problem in undergraduate students. Pain prevalence, pain management strategies and knowledge among healthcare groups has not been revealed yet.
Aim: This study explored pain prevalence, intensity, pain management strategies, knowledge, and education in undergraduate students specializing in healthcare science. The findings will highlight the necessity for increasing pain management education in the university setting. Design: A questionnaire-based, cross-sectional study was conducted.
Settings/ Participants: Data was collected from 1,490 university students in Tokyo between December 2015 and April 2016. A c square test was performed to examine differences in pain status and management strategies according to gender. We compared medical knowledge scores among disciplines using one-way analysis of variance.
Results: In total, 511 (79.2%) students had experienced bodily pain during the preceding 6 months. Pain prevalence differed by gender. More nursing students had used both pharmacological and nonpharmacological methods for pain management than had students from other disciplines (p ¼ .011). Pain medication knowledge of students in other disciplines was low to moderate, with greater knowledge observed in medical students (p < .05).
Conclusions: Education regarding pain management should be developed that considers differences among disciplines. Additionally, poor pain management knowledge could affect the quality of care students provide to patients after graduation. Enhancing pain management knowledge by providing suitable pain management education in universities may contribute to better pain management for students, and this may translate to their work in clinical settings.

8 Pain Management Knowledge of Nurses Working in Northwest Amhara Referral Hospitals in Ethiopia During 2018
مدیریت دانش درد پرستاران شاغل در بیمارستان های ارجاع شمال غربی آمین در اتیوپی در سال 2018-2021
Background: Pain is an unpleasant emotional and sensory experience that is associated with actual or potential tissue damage. Providing comfort and relief of pain of clients are the fundamental role of nurses in nursing practices. However, inadequate knowledge of appropriate pain management has been reported to be a major obstacle to implementing effective pain management by nurses.چ
Objective: The objective of this study was to assess knowledge of pain management techniques in nurses working in referral hospitals in northwest Ethiopia.
Methods: An institution-based, cross-sectional, interviewer-administered questionnaire study of the pain treatment knowledge of 411 nurses was conducted in 2018 in Northwest Referral Hospitals in Ethiopia. The sample size was allocated to each selected referral hospital. A systematic sampling technique was used to select study participants. The descriptive data were presented in frequency tables. Binary and multivariable logistic regression analyses were undertaken to identify associated factors to pain management knowledge of nurses. Variables with a P value < 0.05 were considered as a significant variable.
Results: Only 40.6% of nurses were judged to have had adequate knowledge regarding pain management. Number of years of training (adjusted odds ratio = 2.19; 95% CI, 1.39–3.44), prior pain education (adjusted odds ratio = 2.34; 95% CI, 1.45–3.8), and professional rank (adjusted odds ratio = 3.09; 95% CI, 1.37–6.96) were associated factors for pain management knowledge of nurses.
Conclusions: The level of many nurses’ knowledge of pain management techniques were inadequate. Lack of pain training in their institution, lack of pain education in their academic curriculum, and professional rank were predictors of these nurses’ pain management knowledge. Providing pain management training and employing higher service rank nurses are likely to result in increased nurses’ knowledge of proper pain management techniques.

9 The role of experiential knowledge in hospital nurses’ management of pain-related agitation in people with dementia: an expert performance simulation study
نقش دانش تجربی در مدیریت بیقراری مرتبط با درد در بیماران مبتلا به زوال عقل توسط پرستاران بیمارستان: یک مطالعه شبیه‌سازی عملکرد خبره-2021
Background
Pain-related agitation in hospital patients with dementia presents a diagnostic challenge as patients often cannot explain their agitation. Generally, a deductive process is required of staff, culminating in an analgesic trial. However, evidence suggests the deductions of hospital nurses may be flawed because they may not associate agitation in dementia with painful conditions, thereby missing key clinical cues. While analytical rule-based deduction applies explicit formal knowledge, cognitive scientists argue that tacit experiential knowledge, i.e., the unconscious classification of available cues, is equally important and is always involved.
Aim
To identify the cognitive characteristics unique to nurses who accurately recognize pain-related agitation in a patient with dementia compared with nurses who do not.
Methods
In this descriptive multivariate study, registered acute-care nurses undertook an original computerized virtual simulation requiring them to identify and manage pain-related agitation in a standardized patient with dementia. Process tracing methods captured nurses’ simulation performances alongside their retrospective accounts of thinking. These were correlated with demographic characteristics related to seniority, workplace, training, experience and knowledge captured on a questionnaire. Dual processing theory enabled interpretation of intuitive and analytical cognitive processes.
Findings
Registered medical and surgical nurses (N=274) participated from 10 hospitals. Although formal knowledge about pain in dementia was high (88%), only 13(4.7%) nurses identified pain-related agitation from an injury. These individuals took the longest and used the most cues, undertaking a detailed deductive search. Their recognition of pain-related cues demonstrated accurate experiential knowledge while another 16 nurses identified a fracture without linking the injury to agitation. Over three quarters (78%) of nurses decided on initial antipsychotic treatment. They were quick to decide, using the fewest cues, suggesting agitation was recognized as typical and familiar, with the solution well-known (albeit ineffective). Independent of other variables, nurses working in dementia-specific units, surgical units or with more seniority had increased odds of recognizing pain, revealing the influence of workplace experience. However, most surgical and dementia-unit nurses did not recognize pain.
Conclusions
Hospital nurses have difficulty recognizing when agitation in a patient with dementia is caused by pain. High formal knowledge about pain in dementia may not be sufficient to enable clinical recognition of pain in patients. Instead, nurses with experience in specific workplaces or senior roles may be better equipped to recognize pain-related agitation and deploy evidence-based approaches for agitation in a patient with dementia. Overall, these results lend support to the influence of experiential knowledge on performance.
Tweetable abstract
In a virtual simulation, registered nurses with high formal knowledge about pain in dementia lacked the experience required to recognize pain-related agitation in a patient with dementia

10 Development and validation of INTENSS, a need-supportive training for nurses to support patients self-management
توسعه و اعتبار سنجی های آمیز، آموزش نیازمندی برای پرستاران برای حمایت از خود مدیریت بیماران-2021
Background: The growing prevalence of chronic illnesses requires nurses to support self-management and help patients integrate the chronic illness into their life. To our knowledge there are currently no training programs that combine the necessary components to adequately enhance nurses competencies in self-management support. Objective: The systematic development and validation of a need-supportive training in self-management support for nurses. Design: A three-phased study, according to van Meijel et al. (2004), with collection of building blocks, design, and validation of the need-supportive character of the training. Setting and participants: Eight training groups with 30 nurses, 34 nursing students and nine social healthcare workers from different nursing colleges in Flanders, Belgium. Methods: In phase one a literature review, current practice analysis, and problem and needs analysis were per- formed. In phase two, the INTENSS training intervention was developed, framed within the Self-Determination Theory and the 5A’s-Model. The training consisted of a basic training module and a video-interaction guidance module. The intervention was subsequently tested in eight training groups (N = 73). Participants provided feedback during focus group discussions. The intervention was cyclically adapted to trainees experiences and suggestions. In phase three, we evaluated the need-supportive character of the training intervention. Results: Phase one indicated the need for training, since nurses application of self-management support was limited and practiced from a narrow medical point of view. In phase two we developed a theory-driven and multifaceted training, building on attitude, knowledge, skills and reflection in the training. The training was framed within the Self-Determination Theory both at the didactical level as well as on content and format. Overall, participants appreciated the building blocks of the training as supporting their basic needs for auton- omy, relatedness and competence. Conclusions: INTENSS, a multifaceted need-supportive training in self-management support was developed, successfully taking into account participants needs.
