and therapeutic possibilities are responsible for an increase and a diversification of available data. Big data
offers the capacities, in the field of health, to accelerate the discoveries and to optimize the management of
patients by combining a large volume of data and the creation of therapeutic models. In radiotherapy, the
development of big data is attractive because data are very numerous et heterogeneous (demographics,
radiomics, genomics, radiogenomics, etc.). The expectation would be to predict the effectiveness and
tolerance of radiation therapy. With these new concepts, still at the preliminary stage, it is possible to
create a personalized medicine which is always more secure and reliable.

نانوذرات طلا برای کاربرد پرتودرمانی سرطان: مکانیزیم ها و پیشرفتهای اخیر

Management of glioblastoma in elderly patients
مدیریت گلیوبلاستوما در بیماران مسن-2017 Glioblastoma (GBM) is the most common primary malignant brain tumor in adults over
55 years of age. The median age of diagnosis for patients with GBM is 64 years old, with
the incidence of patients between 75 and 85 increasing. The optimal treatment paradigm
for elderly GBM patients continues to evolve due to the higher frequency of age-related
and/or medical co-morbidities. Geriatric GBM patients have historically been excluded
from larger, controlled clinical trials due to their presumed decreased likelihood of a
sustained treatment response and/or a prolonged good outcome. Here, we highlight
current treatment considerations of elderly GBM patients with respect to surgical,
radiological and systemic modalities, with considerations for improving future clinical
outcomes for this patient population.

Patient characterization and usage trends of proton beam therapy for localized prostate cancer in the United States: A study of the National Cancer Database

external beam radiation therapy (RT) (EBRT) for localized prostate cancer.
Patients and Methods: The National Cancer Database was queried for men with localized (N0, M0) prostate cancer diagnosed between
2004 and 2013, treated with EBRT, with available data on EBRT modality (photon vs. PBT). Binary multiple logistic regression identified
variables associated with EBRT modality.
Results: In total, 143,702 patients were evaluated with relatively few men receiving PBT (5,709 [4.0%]). Significant differences in
patient and clinical characteristics were identified between those men treated with PBT compared to those treated with photon (odds ratio
[OR]; 95% CI). Patients treated with PBT were generally younger (OR ¼ 0.73; CI: 0.67–0.82), National Comprehensive Cancer Network
low-risk compared to intermediate (0.71; 0.65–0.78) or high (0.44; 0.38–0.5) risk, white vs. black race (0.66; 0.58–0.77), with less
comorbidity (Charlson-Deyo 0 vs. 2þ; 0.70; 0.50–0.98), live in higher income counties (1.55; 1.36–1.78), and live in metropolitan areas
compared to urban (0.21; 0.18–0.23) or rural (0.14; 0.10–0.19) areas. Most patients treated with PBT travelled more than 100 miles to the
treatment facility. Annual PBT utilization significantly increased in both total number and percentage of EBRT over time (2.7%–5.6%;
P o 0.001). PBT utilization increased mostly in men classified as National Comprehensive Cancer Network low-risk (4%–10.2%).
Conclusion: PBT for men with localized prostate cancer significantly increased in the United States from 2004 to 2013. Significant
demographic and prognostic differences between those men treated with photons and protons were identified. r 2017 Elsevier Inc. All

Impact of Using Audit Data to Improve the Evidence-Based Use of Single-Fraction Radiation Therapy for Bone Metastases in British Columbia

Methods and Materials: In 2012, an audit of radiation therapy prescriptions for bone metastases in British Columbia identified significant interphysician and -center (26%-73%) variation in the use of SFRT. Anonymous physician-level and identifiable regional cancer center SFRT use data were presented to all radiation oncologists, together with published guidelines, meta-analyses, and recommendations from practice leaders. The use of SFRT for bone metastases from 2007 through 2011 was compared with use of SFRT in 2013, to assess the impact of the audit and educational intervention. Multilevel logistic regression was used to assess the relationship between the usage of SFRT and the timing of the radiation while controlling for potentially confounding variables. Physician and center were included as group effects to account for the clustered structure of the data. Results: A total of 16,898 courses of RT were delivered from 2007 through 2011, and 3200 courses were delivered in 2013. The rates of SFRT use in 2007, 2008, 2009, 2010, 2011, and 2013 were 50.5%, 50.9%, 48.3%, 48.5%, 48.0%, and 59.7%,respectively (P<.001). Use of SFRT increased in each of 5 regional centers: A: 26% to 32%; B: 36% to 56%; C: 39% to 57%; D: 49% to 56%; and E: 73% to 85.0%. Use of SFRT was more consistent; 3 of 5 centers used SFRT for 56% to 57% of bone metastases RT courses. The regression analysis showed strong evidence that the usage of SFRT increased after the 2012 intervention (odds ratio 2.27, 95% confidence interval 2.06-2.50, P<.0001). Conclusion: Assessed on a population basis, an audit-based intervention increased utilization of SFRT for bone metastases. The intervention reversed a trend to decreasing SFRT use, reduced costs, and improved patient convenience. This suggests that dissemination of programmatic quality indicators in oncology can lead to increased utilization of evidence-based practice. 2016 Elsevier Inc. All rights reserved. |

Pathology and cancer policy
آسیب شناسی و خط و مشی سرطان-2016 Pathology is crucial to the investigation and management of the cancer patient. While the diagnostic
importance might be most obviously seen in cell/anatomic pathology, the other pathology specialities
are also vital in, for example, the successful pre- and post-operative management of the cancer
patient where the need for good quality blood transfusion, microbiology/immunology and blood
chemistry to manage, for instance, the complications of chemo- and radiation therapy is obvious.
Having said this, cell pathology has a particular role and will be the focus of the rest of this paper.
At its most basic, cell pathology, using simple haematoxylin and eosin stains, determines the
answers to questions such as: is this mass neoplastic or, for instance, inflammatory? If a neoplasm,
is it benign or malignant? If malignant, what type of malignant tumour is it? How aggressive is it?
How far has it invaded? Has it been fully excised? Has it metastasized to lymph nodes and/or is it
in blood vessels? How many lymph nodes are involved and where located? Using immunohistology,
what is the molecular phenotype? Using nucleic acid analysis, what is the genotype? All of these
features tell the clinician not just the diagnosis, but the likely prognosis, the most appropriate therapy
(especially important in personalized/precision medicine), what follow-up is advisable (eg further
surgery if an excision margin is too close to the tumour), what further investigations might be needed,
and so on. |
