A gold nanocluster chemical tongue sensor array for Alzheimer’s disease diagnosis

heavy financial burden on our society. The use of serologic biomarkers is an attractive method to diagnose AD.
Although the determination of blood-based biomarkers for AD has been explored in many studies, few practical
diagnosis methods have been used in the clinic. In this work, we constructed a “chemical tongue” sensor array
that is easy to use and based on four kinds of fluorescent gold nanoclusters (Au NCs) for discriminating between
multiple proteins at nanomolar concentrations. The device utilizes a linear discrimination analysis based on
fluorescence intensity response patterns. Using this chemical tongue sensor array, multiple proteins can be
confidently identified even in complex biological systems, such as human urine. Most importantly, sera of AD
patients could be effectively discriminated from those of osteoarthritis patients, or of healthy people. Also, the
results obtained for the AD patients by the chemical tongue sensor array were validated by CSF determination.
We conclude that the chemical tongue sensor array manufactured in this work paves the way for designing an
auxiliary diagnosis method for AD that is less invasive and more convenient for the large-scale screening of

Artificial Intelligence and Machine Learning in Lower Extremity Arthroplasty: A Review

and machine learning (ML) applications are poised to expand orthopedic surgery frontiers. Lower extremity
arthroplasty is uniquely positioned to most dramatically benefit from ML applications given its
central role in alternative payment models and the value equation.
Methods: In this report, we discuss the origins and model specifics behind machine learning, consider its
progression into healthcare, and present some of its most recent advances and applications in arthroplasty.
Results: A narrative review of artificial intelligence and ML developments is summarized with specific
applications to lower extremity arthroplasty, with specific lessons learned from osteoarthritis gait
models, joint-specific imaging analysis, and value-based payment models.
Conclusion: The advancement and employment of ML provides an opportunity to provide data-driven,
high performance medicine that can rapidly improve the science, economics, and delivery of lower

Development of Machine Learning Algorithms for Prediction of Sustained Postoperative Opioid Prescriptions After Total Hip Arthroplasty

prolonged opioid use but there are few tools for predicting this adverse outcome. The purpose of this
study is to develop machine learning algorithms for preoperative prediction of prolonged opioid prescriptions
after THA.
Methods: A retrospective review of electronic health records was conducted at 2 academic medical
centers and 3 community hospitals to identify adult patients who underwent THA for osteoarthritis
between January 1, 2000 and August 1, 2018. Prolonged postoperative opioid prescriptions were defined
as continuous opioid prescriptions after surgery to at least 90 days after surgery. Five machine learning
algorithms were developed to predict this outcome and were assessed by discrimination, calibration, and
decision curve analysis.
Results: Overall, 5507 patients underwent THA, of which 345 (6.3%) had prolonged postoperative opioid
prescriptions. The factors determined for prediction of prolonged postoperative opioid prescriptions
were age, duration of opioid exposure, preoperative hemoglobin, and preoperative medications (antidepressants,
benzodiazepines, nonsteroidal anti-inflammatory drugs, and beta-2-agonists). The elasticnet
penalized logistic regression model achieved the best performance across discrimination
(c-statistic ¼ 0.77), calibration, and decision curve analysis. This model was incorporated into a digital
application able to provide both predictions and explanations (available at https://sorg-apps.shinyapps.
io/thaopioid/).
Conclusion: If externally validated in independent populations, the algorithms developed in this study
could improve preoperative screening and support for THA patients at high risk for prolonged postoperative
opioid prescriptions. Early identification and intervention in high-risk cases may mitigate the
long-term adverse consequence of opioid dependence.

Preoperative Prediction of Value Metrics and a Patient-Specific Payment Model for Primary Total Hip Arthroplasty: Development and Validation of a Deep Learning Model

and predicts length of stay (LOS), inpatient charges, and discharge disposition for total hip arthroplasty.
The secondary objective was to create a patient-specific payment model (PSPM) accounting for patient
complexity.
Methods: Using 15 preoperative variables from 78,335 primary total hip arthroplasty cases for osteoarthritis
from the National Inpatient Sample and our institutional database, an ANN was developed to
predict LOS, charges, and disposition. Validity metrics included accuracy and area under the curve of the
receiver operating characteristic curve. Predictive uncertainty was stratified by All Patient Refined comorbidity
cohort to establish the PSPM.
Results: The dynamic model demonstrated “learning” in the first 30 training rounds with areas under the
curve of 82.0%, 83.4%, and 79.4% for LOS, charges, and disposition, respectively. The proposed PSPM
established a risk increase of 2.5%, 8.9%, and 17.3% for moderate, major, and severe comorbidities,
respectively.
Conclusion: The deep learning ANN demonstrated “learning” with good reliability, responsiveness, and
validity in its prediction of value-centered outcomes. This model can be applied to implement a PSPM for

A machine learning approach to knee osteoarthritis phenotyping: data from the FNIH Biomarkers Consortium

distinct phenotypes. The purpose of this study was to apply innovative machine learning approaches to
KOA phenotyping in order to define progression phenotypes that are potentially more responsive to
interventions.
Design: We used publicly available data from the Foundation for the National Institutes of Health (FNIH)
osteoarthritis (OA) Biomarkers Consortium, where radiographic (medial joint space narrowing of
0.7 mm), and pain progression (increase of 9 Western Ontario and McMaster Universities Osteoarthritis
Index [WOMAC] points) were defined at 48 months, as four mutually exclusive outcome groups
(none, both, pain only, radiographic only), along with an extensive set of covariates. We applied distance
weighted discrimination (DWD), direction-projection-permutation (DiProPerm) testing, and clustering
methods to focus on the contrast (z-scores) between those progressing by both criteria (“progressors”)
and those progressing by neither (“non-progressors”).
Results: Using all observations (597 individuals, 59% women, mean age 62 years and BMI 31 kg/m2) and
all 73 baseline variables available in the dataset, there was a clear separation among progressors and
non-progressors (z ¼ 10.1). Higher z-scores were seen for the magnetic resonance imaging (MRI)-based
variables than for demographic/clinical variables or biochemical markers. Baseline variables with the
greatest contribution to non-progression at 48 months included WOMAC pain, lateral meniscal extrusion,
and serum N-terminal pro-peptide of collagen IIA (PIIANP), while those contributing to progression
included bone marrow lesions, osteophytes, medial meniscal extrusion, and urine C-terminal crosslinked
telopeptide type II collagen (CTX-II).
Conclusions: Using methods that provide a way to assess numerous variables of different types and
scalings simultaneously in relation to an outcome of interest enabled a data-driven approach that

Rationale and design of the Staying Positive with Arthritis (SPA) Study: A randomized controlled trial testing the impact of a positive psychology intervention on racial disparities in pain

arthritis treatments yield small to moderate improvements in pain and have not been effective at reducing racial
disparities in the management of pain. The biopsychosocial model of pain and evidence from the positive
psychology literature suggest that increasing positive psychological skills (e.g., gratitude, kindness) could im
prove pain and functioning and reduce disparities in osteoarthritis pain management. Activities to cultivate
positive psychological skills have been developed and validated; however, they have not been tested in patients
with osteoarthritis, their effects on racial differences in health outcomes have not been examined, and evidence
of their effects on health outcomes in patients with other chronic illnesses is of limited quality. In this article we
describe the rationale and design of Staying Positive with Arthritis (SPA) study, a randomized controlled trial in
which 180 African American and 180 White primary care patients with chronic pain from knee osteoarthritis
will be randomized to a 6-week program of either positive skill-building activities or neutral control activities.
The primary outcomes will be self-reported pain and functioning as measured by the WOMAC Osteoarthritis
Index. We will assess these primary outcomes and potential, exploratory psychosocial mediating variables at an
in-person baseline visit and by telephone at 1, 3, and 6 months following completion of the assigned program. If
effective, the SPA program would be a novel, theoretically-informed psychosocial intervention to improve
quality and equity of care in the management of chronic pain from osteoarthritis.
