Factors associated with knowledge towards postoperative nausea and vomiting management among health professionals in referral Hospitals of Northwest Ethiopia: A multi-center cross-sectional study

emetic prescription trends affects patient’s outcome after surgery and anesthesia and also patient and family
satisfaction. Hence, knowing the knowledge status of health professionals towards PONV management is vital for
the optimal care of surgical patients. Therefore, the study aimed to assess the knowledge and factors associated
with PONV management among health professionals in referral hospitals of Northwest Ethiopia.
Methods: An institutional based cross-sectional study was conducted on 407 health care professionals from March
1 to 30, 2019. A Simple random sampling technique was used to select the study participants. Both bivariable
and multivariable logistic regression analysis were used to identify factors associated with the knowledge level of
health professionals on PONV management. In the multivariable analysis, variables with a p-value <0.05 were
considered statistically significant.
Results: In this study, about 52.8% (95% CI: 47.9, 57.2) of the participants had good knowledge of PONV
management. Being male (AOR = 1.95; 95% CI: 1.20, 3.17), Physician (AOR = 5.36; 95% CI: 2.20, 13.5),
Anesthetist (AOR = 3.88; 95% CI: 1.66, 9.08), and taking training on PONV management (AOR = 5.32; 95%CI:
1.58, 17.89) were positively associated with good knowledgeable of health professionals about PONV
management.
Conclusion: and recommendation: More than half of health care professionals who are working in the periop-
erative sites of the referral hospitals had good knowledge about the PONV management. Being male, Physician,
Anesthetist and taking in-service training on PONV management were significantly affecting the knowledge level
of health professionals on PONV management. Thus, providing regular in-service training on PONV manage-

Deep-learning model for predicting 30-day postoperative mortality

available prediction tools using summaries of intraoperative data are limited by their inability to reflect shifting risk
associated with intraoperative physiological perturbations. We sought to compare similar benchmarks to a deeplearning
algorithm predicting postoperative 30-day mortality.
Methods: We constructed a multipath convolutional neural network model using patient characteristics, co-morbid
conditions, preoperative laboratory values, and intraoperative numerical data from patients undergoing surgery with
tracheal intubation at a single medical centre. Data for 60 min prior to a randomly selected time point were utilised.
Model performance was compared with a deep neural network, a random forest, a support vector machine, and a logistic
regression using predetermined summary statistics of intraoperative data.
Results: Of 95 907 patients, 941 (1%) died within 30 days. The multipath convolutional neural network predicted postoperative
30-day mortality with an area under the receiver operating characteristic curve of 0.867 (95% confidence interval
[CI]: 0.835e0.899). This was higher than that for the deep neural network (0.825; 95% CI: 0.790e0.860), random forest
(0.848; 95% CI: 0.815e0.882), support vector machine (0.836; 95% CI: 0.802e870), and logistic regression (0.837; 95% CI:
0.803e0.871).
Conclusions: A deep-learning time-series model improves prediction compared with models with simple summaries of
intraoperative data. We have created a model that can be used in real time to detect dynamic changes in a patient’s risk

Acquiring skills in malignant hyperthermia crisis management: comparison of high-fidelity simulation versus computer-based case study

simulation versus a computer-based case solving self-study, in skills acquisition about malignant
hyperthermia on first year anesthesiology residents.
Methods: After institutional ethical committee approval, 31 first year anesthesiology residents
were enrolled in this prospective randomized single-blinded study. Participants were randomized to either a High Fidelity Simulation Scenario or a computer-based Case Study about malignant hyperthermia. After the inter vention, all subjects’ performance in was assessedthrough a high fidelity simulation scenario using a previously validated assessment rubric. Additionally, knowledge tests and a satisfaction survey were applied. Finally, a semi-structured interview was done to assess self-perception of reasoning process and decision-making.
Results: 28 first year residents finished successfully the study. Resident’s management skill
scores were globally higher in High Fidelity Simulation versus Case Study, however they were
significant in 4 of the 8 performance rubric elements: recognize signs and symptoms (p = 0.025),
prioritization of initial actions of management (p = 0.003), recognize complications (p = 0.025)
and communication (p = 0.025). Average scores from pre- and post-test knowledge questionnaires improved from 74% to 85% in the High Fidelity Simulation group, and decreased from
78% to 75% in the Case Study group (p = 0.032). Regarding the qualitative analysis, there was no
difference in factors influencing the student’s process of reasoning and decision-making with
both teaching strategies.

Effect of sleep deprivation after a night shift duty on simulated crisis management by residents in anaesthesia. A randomised crossover study

jeopardise patients’ safety during medical crisis management. The aim of the study was to assess the effect
of sleep deprivation on the management of simulated anaesthesia crisis by residents in anaesthesiology.
Methods: A randomised, comparative, monocentric crossover study involving 48 residents in anaesthesia
was performed on a high fidelity patient simulator. Each resident was evaluated in a sleep-deprived
state (deprived group, after a night shift duty) and control state (control group, after a night of sleep).
Performance was assessed through points obtained during crisis scenario 1 (oesophageal intubation
followed by anaphylactic shock) and scenario 2 (anaesthesia-related bronchospasm followed by
ventricular tachycardia). Sleep periods were recorded by actigraphy. Two independent observers
assessed the performances. The primary endpoint of the study was the score obtained for each scenario.
Results: Resident’s crisis management performance is associated with sleep deprivation (scenario 1:
control = 39 [33–42] points vs. deprived = 26 [19–40] points, P = 0.02; scenario 2: control = 21 [17–24]
vs. deprived = 14 [12–19], P = 0.01). The main errors observed were: error in drug administration and
dose, delay in identification of hypotension, and missing communication with the surgical team about
situation.
Conclusions: The present study showed that sleep deprivation is associated with impairment of
performance to manage crisis situations by residents in anaesthesia.

Retro-peritoneal paraganglioma, diagnosis and management
paraganglioma یکپارچه صفاقی، تشخیص و مدیریت-2018 Introduction. — Paragangliomas, defined as extra-adrenal chromaffin-cells tumors, are rarely
located in the retro-peritoneum. Clinical presentation is similar to pheochromocytoma, and
mainly depends on the producing character of the tumor. Positive diagnosis requires plasmatic
and urinary hormonal assays. Radiological and isotopic explorations are essential before surgery.
The only curative therapeutic strategy is surgical, associated to peri-operative prevention and
monitoring of the frequently reported hemodynamic and cardiovascular disorders. Outcome
depends of the metastatic character of the tumor, the presence of tumor remnant after surgi
cal resection. Genetic study is recommended; the risk of recurrence and association to other
neoplasm is more described in genetic forms.
Material and methods. — Authors report 5 cases of retro-peritoneal paraganglioma, operated
in the department of urology of Hospital, between 2013 and 2017. Observations are about
2 men and 3 women. Clinical presentation is not always specific and paraganglioma may be
discovered fortuitously. Two patients have been operated by coelioscopic approach, midline
incision was performed in two other cases, and dorsal lumbotomy associated to a Rutherford
Morrison incision in a patient.

The use of a checklist improves anaesthesiologists’ technical and non-technical performance for simulated malignant hyperthermia management

emergencies. Managing OR emergencies implies real-time analysis of often complicated situations,
prompt medical knowledge retrieval, coordinated teamwork and effective decision making in stressful
settings. Checklists are recommended to improve performance and reduce the risk of medical errors. This
study aimed to assess the usefulness of the French Society of Anaesthesia and Intensive Care’s (SFAR)
‘‘Malignant Hyperthermia’’ (MH) checklist on a simulated episode of MH crisis and management thereof
by registered anesthesiologists.
Methods: Twenty-four anaesthesiologists were allocated to 2 groups (checklist and control). Their
technical performance in adherence with the SFAR guidelines was assessed by a 30-point score and their
non-technical performance was assessed by the Anaesthetists’ Non-Technical Skills (ANTS) score. Every
task completion was assessed independently. Data are shown as median (first-third quartiles).
Results: Anaesthesiologists in the checklist group had higher technical performance scores (24/30 (21.5–
25) vs 18/30 (15.5–19.5), P = 0.002) and ANTS scores (56.5/60 (47.5–58) vs 48.5/60 (41–50.5), P = 0.024).
They administered the complete initial dose of dantrolene (2 mg/kg) more quickly (15.7 minutes [13.9–
18.3] vs 22.4 minutes [18.6–25]) than the control group (P = 0.017). However, anaesthesiologists deemed
the usability of the checklist to be perfectible.
Conclusion: Registered anaesthesiologists’ use of the MH checklist during a simulation session widely
improved their adherence to guidelines and non-technical skills. This study strongly suggests the benefit
of checklist tools for emergency management. Notwithstanding, better awareness and training for
anaesthesiologists could further improve the use of this tool.

Review of the Society of Thoracic Surgeons Congenital Heart Surgery Database: 2017 Update on Outcomes and Quality Implications for the Anesthesiologist

where data collection and analysis influence medical decision making. After three decades of
data collection, analysis and subsequent changes in clinical management, progress is being
made in developing data-driven care protocols in patients with congenital heart disease.1 The
term standardization in reference to medical care can be misinterpreted as a “one size fits all” mentality. Medical care should be individualized to a specific patient, whereas data analyses
and review of patient outcomes potentially decrease unnecessary variation in care.
Additionally, comparative data has helped influence important concepts such as center
transparency and public reporting in congenital heart surgery. Centers can access data from
the Society of Thoracic Surgeons website since January 2015.
